I found my spare card of the same model and it gives me the same problem.
Here is a list of all the packages I installed, in case I missed something.

inetenbl.pkg
newtdev.pkg
enetsup.pkg
loctsup.pkg
inetstup.pkg
FarallonEthernet.pkg
3comaddon.pkg
3c589.pkg

I believe I have Prefs, Owner Info, and Dock configured correctly.
