Its a good question.
On the "Big" shopping day, I generally use the check list stationary to create my list. I've had the same document for years with only minor changes.
Newt Grocery seems like another good way. http://www.unna.org/view.php?/applications/NewtGrocery1.0
The worst way I was silly enough to try was tallying everything in QuickFigure (spreadsheet) as I shopped. It was just too slow.
