since the first poster had mentioned PDF, Newt's Cape has been used
with various PDF-to-HTML (or PDF-to-text) converters, the results are
usually far from perfect but it is an option. if one hand-edited the
HTML to add <H1> , <H2> ... tags, then table of contents would happen
for free.

it's not documented. it would be easier to create the "NTK format"
that Bookmaker does, and thus eliminate Bookmarker, but you'd still
need a way to compile a package, i.e., NTK.
