You might try upgrading to the newest version (5.2.4) as the release
notes (<http://homepage.mac.com/simonbell/mailv/news_links.html>)
state the following for version 5.2.3 (plus the stuff for 5.2.4):

"May 28, 2004 - Version 5.1.3 does not force a .nwt filename extension
on forwarded attachments. [...]"
