I have to add that in the Palm world there's a program called iSilo that
allows creating indexes and hyperlinks in documents. There's a DSM-IV
document for iSilo that works really great, although you have to buy iSilo
for $15 after the 30-day trial period. After that, the DSM-IV document is
free.
