On second thought, I let my mouse and Google do the driving. That address is a
building called the Toronto Business Development Centre. Described as an
incubator for entrepreneurs; appears to be funded or sponsored by the City.

I guess Stone Multimedia could have moved in since then, but coincidentally
their business sounds similar to Egresk's.
All in all, this "survey" does not look realistic to me. A company with the
kind of money that I assume it would require to even begin to interest Apple in
selling any of its intellectual property, would not likely be getting their
market research done by a startup that can't afford to get their documents
proofread.
Maybe somebody's jumping the gun for April 1st?
