I recall that I used one of the major vendors when I ordered.
Going by the information I recall and specs that Stephanie listed here are
two options for you.
Though I suggest using the Through Hole version.

5.000 mHz Crystal Parallel 18pf

Through Holes (suggested unit as the legs make it easier to solder)

Surface Mount (type that was used originally)

I'd let someone more technical verify this info, but I did use them when
doing my hack to MP2x00 and eMate 300.
